Below is the simple questionnaire. It is a catch-all form which will be used by the Raven Chronicles to generate the Pacific Northwest Writers Index.
Please feel free to provide only the information you feel comfortable to share. Not every writer will have publication listings, but include other activities instead. We are looking for e-mail addresses primarily,but those who do not mind including physical mail addresses, please do so.
You can also attach your photo (or other image) as a gif/jpeg (saved as 200 pixels by 200 pixels) to your e-mail, when you return the completed questionnaire.
