Below is the simple questionnaire. It is a catch-all form which willbe used by the Raven Chronicles to generate the Pacific Northwest WritersIndex.
Please feel free to provide only the information you feel comfortableto share. Not every writer will have publication listings, but include otheractivities instead. We are looking for e-mail addresses primarily,but those who do not mind including physical mail addresses, please do so.
